FNCA FY2012 Workshop on Radiation Oncology Project

The FNCA 2012 Workshop on Radiation Oncology was held from 15 to 18th January 2013 in Bangkok, Thailand. The workshop was co-organized by Thailand Institute of Nuclear Technology (TINT), Siriraj Hospital, Mahidol University and the Ministry of Education, Culture, Sports, Science and Technology (MEXT) of Japan. 27 representatives from 11 FNCA Member States, namely Bangladesh, China, Indonesia, Japan, Kazakhstan, Korea, Malaysia, Mongolia, the Philippines, Thailand and Vietnam, and 3 Regional Cooperation Agreement (RCA) Member States-Myanmar, Pakistan and Sri Lanka as observers participated in the workshop.
This project carries out some international joint clinical studies in order to establish treatment protocols for Uterine Cervix cancer and Nasopharyngeal cancer, which affect large numbers of people in FNCA member countries, and finally to improve the technique of radiation oncology in the Asian region.

Opening Ceremony
Dr. Yaowalak Chansilpa, Associate Professor, Division of Radiation Oncology,
Faculty of Medicine Siriraj Hospital, Mahidol University moderated the session.
Assoc. Prof. Vutisiri Veerasarn, Head, Division of Radiation Oncology, Faculty of Medicine Siriraj Hospital, Mahidol University opened the workshop officially. She reminded the participants about Thailand cooperation in the FNCA project. She also gave welcome remarks and gave her full support for this meeting. Following them, Dr. Hirohiko Tsujii, Former Executive Director of NIRS delivered his remarks.

   

Assist. Prof. Nantakan Apivarodom and Dr. Kullathorn Thephamongkhol , Faculty of Medicine, Siriraj Hospital, Mahidol University gave a special lecture on Breast Cancer. They spoke about rationales and techniques of whole breast irradiation, including 2D, 3D and IMRT. Intraoperative radiotherapy (IORT) for partial breast irradiation in breast cancer was also introduced and the general concepts and controversies were discussed.

   

Phase II Study of Concurrent Chemotherapy and Extended-Field Radiotherapy for Locally Advanced Cervical Cancer (CERVIX-IV)
In this protocol, 83 patients are registered from all the countries (at the point of this workshop) and 79 patients are considered evaluable. (However, the total recent clinical data presented by the representatives of each country in this session was 77, which consisted of Bangladesh (21), China (6), Indonesia (6), Japan (19), Korea (7), Malaysia (5), Philippines (4), Thailand (4) and Vietnam (5).) Out of 83 cases, half of them were stage IIB and another half were stage IIIB. The follow-up rate at the end of 2012 was 99% (78/79 cases). It was decided that recruitment of new cases to continue to reach the target number of 100 cases for this protocol study.

    

Phase II Study of Chemoradiotherapy for NPC (any T, N2-3) NPC-I
The summary of the protocol and follow-up data was presented. The median follow-up time is 46 months for the surviving 77 / 121 patients. The 3 year local regional control and overall survival rates were 89% and 66% respectively. It was reported that the NPC-I trial result was published in the Journal of Radiation Research, 2012. In addition, the reviewers' comments were discussed. It was highlighted that the paper for future publication should emphasize on the challenges of conducting multi-center trials in Asian countries which have different resources and constraints.

   

Phase II Study of Chemoradiotherapy for NPC (T3-4, N0-1) (NPC-II)
A total of 74 patients are registered in this protocol. (However, the total recent clinical data presented by the representatives of each country in this session was 70, which consisted of Bangladesh (0), China (0), Indonesia (13), Japan (0), Korea (0), Malaysia (6), Philippines (0), Thailand (1) and Vietnam (54)). This was followed by the open discussion which centered on why locoregional control was lower than expected when compared to sub group T3-4, N0-1, in NPC I protocol. Several hypotheses were suggested, e.g. RT technique differences (2D vs 3D), adjuvant chemo or natural history of subgroup.

   

Country Report from New Member Country and Collaboration of FNCA and IAEA/RCA
Country Reports were presented by Prof. Tasbolat Adylkhanov, a representative from Kazakhstan (a new FNCA member country) and Dr. Khin Cho Win from Myanmar. Ms. Masooma Riaz from Pakistan presented on Incidence and Treatment of Breast and Cervical Cancer. Current treatment of NPC in Sri Lanka was presented by Dr. M. A. Yasantha Ariyaratne from Sri Lanka.

Phase I/II Study of Concurrent Chemoradiotherapy (CCRT) for NPC (NPC-III)
The recent clinical data of NPC-III with 41(29 males and 12 females) registered patients was presented. (However, the total recent clinical data presented by the representatives of each country in this session was 37, which consisted of Bangladesh ( 1 ), China ( 4 ), Indonesia ( 6 ), Japan ( 0 ), Korea ( 0 ), Malaysia ( 9 ), Philippines ( 7 ), Thailand ( 0 ) and Vietnam ( 10 ).) The summary of the clinical data and evaluation of the clinical study results were reported. 100% of patients received induction chemotherapy and 73% received CCRT (at least 4 cycles of chemotherapy). There was interruption of CCRT in 17 patients (41%) and the toxicity was milder compared to NPC-I. So far, most countries found the protocol tolerable and that more cases are expected to be enrolled.

   

New Protocols
The new protocols were suggested by Japan with the termination of 2 protocols, Cervix-III and NPC-I.
First, overview and the purpose of this session were explained.
Next, a presentation on Cervical V study proposal was given. Three clinical studies were presented and discussed as follows.

(A) 3D image-guided Brachytherapy (IGBT).
The objectives, primary and secondary endpoints and outlines of the study were presented and discussed.
(B) Phase II study of CCRT for locally advanced adenocarcinoma.
Since the number of cases is low, it was agreed to consider this study at future meeting.
(C) Randomized clinical study comparing CCRT with or without prophylactic para-aortic radiotherapy for patients with locally advanced squamous cell carcinoma of the cervix with pelvic lymph node metastasis.

The possibility of the randomized clinical study was discussed. All present agreed that we will proceed to this protocol study after the completion of Cervix IV.

    

Another presentation on Breast Cancer using hypofractionated irradiation for breast conserving surgery and post mastectomy patients was made. It was emphasized that the goal of this clinical study is to confirm the safety and efficacy of this regimen in comparison to conventional fractionation and superior in terms of convenience and cost effectiveness. The participants agreed on the study protocol and to start the recruitment after receiving final detail on study protocol.
Suggestions from the participants will be incorporated and distributed after this meeting is closed.

QA/QC of External Beam Thrapy
This activity aims to put in place reliable dosimetry in the institutes among the member countries for effective joint clinical studies. The audits in QA/QC of dosimetry measurement and radiation calibration have been conducted, which is for the reliable radiotherapy of cervical cancers.
A report on a summary of intercomparison measurements using validated glass dosimeters done on 42 beams in 15 facilities with 4-18 MV at 10 centers were presented by Japan. This fiscal year, measurements were done in National Cancer Hospital, Vietnam. The results were within 1% and turned out to be accepatable. Future plan is to do additional audit in Kazakhstan and Myanmar and possibly to include new member countries.

Thailand also spoke about QA for Advanced Technique in Radiation Therapy, which included QA/QC needs, techniques, methods and radiotherapy error types occurring frequently in external beam radiation therapy.

    

Technical Visit
The participants conducted a Technical Visit to Siriraj Hospital, Mahidol University. The background and achievements of the hospital were presented. This was followed by a tour of the radiotherapy facilities of the hospital. A presentation on hospital based Cancer Registry was given.
The participants also visited Siriraj Piyamaharajkaroon Hospital (SIPH). After observation of the hospital, a presentation on Thai traditional medicine was given.

    

Future Plan, Other Activities
It was agreed that the next workshop will be held in Korea tentatively from 25-30 November, 2013 (first choice) or 18-23 November, 2013(second choice). The participants were reminded that they should report the results of FNCA protocol studies in future presentations in conferences.

    

Closing Session
The workshop was closed officially with Dr. Hirohiko Tsujii's remark. He expressed his appreciation to Thailand as well as all the participants for their contribution and his expectations for the future of the project.

Open Lecture
Lectures open to the public was held at Jasmine Executive Suite as a part of the workshop. There were 5 lectures on the themes of 1) Introduction of FNCA Radiation Oncology Project, 2) Image Guided Adaptive Brachytherapy (IGAB), 3) Stereotactic Body Radiotherapy (SBRT), 4) Particle Beam Irradiation and 5) Cyber Knife.
There were around 60 participants including medical doctors, university officials, students and public.

(1) Following the agreement of Forum for Nuclear Cooperation in Asia (FNCA) /the 13th FNCA Coordinators Meeting, the FNCA FY2012 Workshop on Radiation Oncology was held from January 15th to 18th, 2013, in Bangkok, Thailand. The meeting was co-organized by Thailand Institute of Nuclear Technology (TINT), Siriraj Hospital, Mahidol University and the Ministry of Education, Culture, Sports, Science and Technology (MEXT) of Japan. Representatives from 11 FNCA Member States, namely Bangladesh, China, Indonesia, Japan, Kazakhstan, Korea, Malaysia, Mongolia, the Philippines, Thailand and Vietnam and 3 Regional Cooperation Agreement (RCA) Member States - Myanmar, Pakistan and Sri Lanka as observers from the International Atomic Energy Agency (IAEA) participated in the meeting.

(5) The agenda was adopted, and chairpersons and reporters were elected. (See Annex 1)

(6) Prof. Shingo Kato, Professor of the Department of Radiation Oncology, International Medical Center, Saitama Medical University introduced the protocol of Cervix IV. The summary of the follow-up data was also presented by him. 83 cases were registered in this protocol, half of them were stage IIB and another half were stage IIIB. The follow-up rate at the end of 2012 was 99% (78/79 cases). This fiscal year, measurements were done in National Cancer Hospital, Vietnam. Future plan is to do additional audit in Kazakhstan and Myanmar and possibly to include new member countries.

(20) Assist. Prof. Chumpot Kakanaporn, spoke about QA for Advanced Technique in Radiation Therapy, which included QA/QC needs, techniques, methods and radiotherapy error types occurring frequently in EBRT.

Session 8: Technical Visit at Siriraj Hospital, Mahidol University

(21) The participants conducted a Technical Visit to Siriraj Hospital Mahidol University. Prof. Chokechai Maetheetrirat, the Medical Director welcomed the group and presented the background and achievements of the hospital. The participants were very impressed. This was followed by a tour of the radiotherapy facilities of the hospital. A presentation on hospital based Cancer Registry was given by Associate Professor Suvanit Theerasakvichaya.

Session 9: Technical Visit at Siriraj Piyamaharajkaroon Hospital

(22) The participants conducted a Technical Visit to Siriraj Piyamaharajkaroon Hospital (SIPH). In the afternoon, the participants visited the Department of Thai Traditional Medicine. Associate Professor Tawee Laohapond gave a presentation on Thai traditional medicine.

Session 10: Future Plan, Other Activities

(23) Regarding Next Workshop, Schedule of the Next Workshop and Other activities, the following items were discussed and agreed.

1. We agreed that the next workshop will be held in Korea, tentatively on 25 - 30 November, 2013 (first choice) or 18 - 23 November, 2013 (second choice).
2. Dr. Tsujii reminded the participants to use results of FNCA protocol studies in future presentations in conferences.
3. He also emphasized the importance of continuing participations of IAEA member states.
4. It was agreed by participants that for future meetings, the representatives from FNCA and IAEA should be the same person nominated from the country to ensure continuity and success of the projects.

Session 11: Drafting the Workshop Minutes

(24) The draft minutes presented by reporters were discussed and amended. The minutes were unanimously adopted by the workshop participants.

(25) Dr. Hirohiko Tsujii closed the workshop officially with his closing remark, in which he expressed his appreciation to Thailand as well as all the participants for their contribution and his expectations for the future of the project.

(26) Open lecture to the public was held at Jasmine Executive Suite as a part of the workshop. In Opening Ceremony, Assoc. Prof. Dr. Prasert Lertsanguansinchai, Chairman, Thai Society of Therapeutic Radiology and Oncology gave a welcome remark. Miriam Joy. C. Calaguas, Chairperson, St.Luke's Medical Center/Jose Reyes Memorial Medical Center and Beena Devi, Senior Consultant Clinical Oncologist, Department of Radiotherapy, Oncology & Palliative Care Sarawak General Hospital moderated the session.

(27) As the first lecture, Dr. Kumiko Karasawa from NIRS spoke on Introduction of FNCA Radiation Oncology Project.

(28) On the theme of Image Guided Adaptive Brachytherapy (IGAB), Prof. Shingo Kato from Saitama Medical University gave a lecture.

(29) On the theme of Stereotactic Body Radiotherapy (SBRT), Prof. Yoshihiro Takai, Professor, Department of Radiology and Radiation Oncology Hirosaki University School of Medicine gave a lecture.

(30) Prof. Tatsuya Ohno from Gunma University, Heavy Ion Medical Center, gave a lecture on Particle Beam Irradiation.

(31) For the last lecture, Dr. Chul Koo Cho, Director General, Korea Institute of Radiological & Medical Sciences (KIRAMS), Korea Cancer Center Hospital (KCCH) on the theme of Cyber Knife.
